Three Former Parkside Manor Employees Charged For Freezing Death Of 89-Year-Old Woman

A Kenosha woman, her daughter and a Racine man have all been charged in connection with the December 18, 2022 death of an 89-year-old woman. The woman was a resident of Parkside Manor, a residential facility located at 6300 67th St in Kenosha. According to the chief medical examiner, Dr. Wieslawa Tlomak, the woman died of hypothermia due to environmental cold exposure. The autopsy findings took an abnormally long amount of time, becuase the toxicology results weren’t available until approximately September 13, 2023 – almost a year after the death.

The woman was found deceased by Kenosha Police on December 18, 2022 at approximately 7:45 am outside of the facility on the sidewalk. Demontae M. Collins, 35, of Racine, WI, Susan Valentin, 40, of Kenosha, WI, and her daughter, Liliana B. Lozano, 20, of Racine, WI were all charged on Wednesday December 6, 2023 with one count of Recklessly Abusing a Patient, Causing Death, a class C felony. If convicted, the trio face up to 40 years in the Wisconsin Prison system.

According to the criminal complaint, all three defendants actions and inactions all played a role in the woman’s death. Routine checks were not performed, alarms were ignored and silenced, and at least two of the defendants left the building for long periods of time, leaving the other two alone with all of the patients for the graveyard shift. Collins, allegedly left the facility for a couple of hours at a time.

“[Kenosha Police] Detective Correa reported that based on this investigation and statements, a number of policies and safeguards for the residents were not followed. Defendant Valentin and defendant Lozano were not notified by defendant Collins left for his “break.” This left the residents in the memory care unit, who have various individualized service plants with various needs unattended for over two and a half hours, which created an unreasonable risk of harm to the residents. Defendant Collins did not conduct normal 1-2 hour checks on residents to confirm that there were no concerns. Had these checks been done [the victim] would have been found to not be in her room. None of the defendants contacted management after the alarm went off. None of the defendants checked the outside of the facility. None of the defendants wrote a report about the fire alarm sounding. Neither defendant Valentin or defendant Lozano responded to the audible fire alarm in memory care for 28 minutes. Once defendant Valentin responded to the alarm, it was then noticed that defendant Collins was not in the facility at the memory care unit. Defendant Valentin notified defendant Lozano approximately 4 minutes later and turned the alarm off with a key. Defendants Lozano and Valentin assumed another resident was the one who set the alarm off, and did not follow protocols by checking on the residents to confirm they were all accounted for. Defendants Lozano and Valentin did not open the fire exit door to check outside, nor was a walk around of the building conducted to confirm no residents were outside. This is in disregard of proper protocols to check on residents to determine they were present after the fire alarm, and compromised the safety of the residents, including [the victim]. The temperature at this time was approximately 9 degrees Fahrenheit, which causes an unreasonable risk of harm, or death. Based on the fact that [the victim’s] bed was made and she was wearing the same clothing as observed by her son and daughter in law the day before, it appears as if [the victim] was never checked on to see if she was in bed at any point in the night… The three employees on duty at Parkside Manor, defendant Collins, defendant Valentin, and defendant Lozano, all failed to follow these policies and procedures to safeguard the residents at the facility. As a result of their failure to follow these policies and procedures, [the victim] exited the building, and was unable to re-enter the facility in single digit cold, resulting in her death.”

All three defendants are currently wanted and KCE is unaware if they are in custody. The jury instructions and elements of the alleged crime describe an uphill battle for prosecutors.

  1. They don’t deserve any mercy. Hopefully they’ll get the maximum penalties. Hopefully our ADAs won’t be offering ridiculous plea deals this time. An example should be set Nationwide. How many elderly are at the mercy of human turds like these?
    The owners of the place sold it immediately after the infamous incident took place, hopefully the family will sue them for all the money they can still recover.

    1. I feel so bad for the older people that’s in these nursing homes that don’t even care about their well-being. I have a grandmother that’s at a nursery home and Kenosha also and they are very very disrespectful. Don’t check on her not Clean feeding her all type of spoiled milk in herfeeding tube don’t answer phones disrespectful when you come and visit make you feel uncomfortable and got my grandmother looking so bad and I called everyone and did everything in my power to try to get her out of there and what do I get she still there been there for three months it’s so sad and sick I cry every night because of this and the state or police don’t understand that theses older people need real people to care for them not just for a check man lm praying for them and my grandma because she doesn’t really understand what going on🥲

It was the man, Demontae Collins, who was in charge of the memory care unit, he left on a “break” for 2-1/2 hours without informing his co-workers, he is the one who had numerous domestic violence charges. How is a person with such a history hired and put in charge of vulnerable, elderly people? Ms. Valentin and Ms. Lozano are culpable as well, no doubt about that, but he was first in charge of that unit and totally abdicated his responsibility.

      Other news sites report surveillance video showing Ms. Ende going out through the fire door and then later making attempts to get back inside. Later, inside the building, video shows the employees going near the fire door but not opening it to look outside, let alone actually going out to look around. The temperature was 9 degrees.

      How the facility’s owners were only fined $2,000 for this mystifies me.

  5. Unless any of them have some kind of past legal stuff like probation that might come into play they system won’t do much of anything.
    At least one of them was probably grazing the food supply and who knows what the other ones were doing on extended break outside the facility but don’t expect any sort of legal or moral responsibility to be demanded of these scum balls.
    Maybe some day, if they survive their way of life, they will be under the care of some attendant who duplicates the same attention to elder care as they did.

    1. The two women probably were on the assisted living section, which is separate from the memory care. Do you think this was the first time Demontae Collins left for an extended “break” in the middle of the night? He lives in Racine. My guess is “break” consisted of a booty call or something more nefarious.

      I agree that not much of a sentence will be imposed on anybody. How about requiring restitution for funeral expenses and counseling or other treatment that the victim’s family may have needed after this happened?

I reported it to the regional manager and the state and I know for a fact I wasn’t the only one who reported the foul shit that was happening and i want to mention families of the residents were also reporting signs of neglect it’s sad that someone had the freeze to death for something to happen

    1. Most ALF’s don’t require staff to be CNA certified, some don’t even mandate CBRF training, or they will offer it on the job. They will hire any warm body for 3rd shift in these facilities. Wages are usually somewhere between $12-$14/hr, with small shift differentials. Several years ago (2015) while working on my MSN, I worked there as a caregiver (I did the Marquette direct MSN) and they were VERY top-heavy, and the right hand didn’t know what the left hand was doing. I worked there at the time that the caregiver was fired for taking naked pictures of one of the residents and putting them on social media. Administration was a disaster, the only person who had it together was Sarah, the ED, and after she left, the place just circled the drain.

      This is sadly unsurprising. For the wages they offer, they’re not going to be attracting quality. Our elderly population truly deserves better.

  8. Encore knew what was happening in that building staff were reporting neglect and abuse and nothing was ever done. The ed hired friends and family that refused to work and when it was brought up to the ed you were a target for her she made sure to make your shift hell and when it was brought to the attention the regional vice president of operations she would respond with I will look into it.

      1. Somebody almost as bad as the one before. One resident (my family member) was taken to the ER and they did not notify anyone in the family. We only found out because that family member called us when she was doing okay. There was no report about it.

        Items are still being stolen, resident aren’t receiving their meals, the list goes on and on. We’ve reported them to various places several times but nothing’s happened. We want to get the family member out but other places around here are just as bad or so expensive we can’t afford it. It’s a terrible situation and the residents are the ones suffering the most.
